What is needed for the reissue or renewal of the passport of a citizen of Ukraine, Hanna SAMSONOVA, a head of the department for the provision of administrative services of the Volnovakha Regional State Administration, says.

If it is necessary to reissue or renew a passport of a citizen of Ukraine of the 1994 sample (in booklet form) or the modern sample (in the form of a credit card), it is necessary to contact the territorial subdivision of the State Migration Service or the administrative service center according to the registered place of residence.

Failure to submit the complete list of documents or failure to comply with the requirements of the legislation may result in the applicant’s refusal to accept the documents and the application form.

What do you need to do if your passport in booklet form is lost?

To obtain a passport of a citizen of Ukraine it is necessary to submit:

1) application form;

2) a written statement on the loss or theft of the passport;

3) an extract from the Unified Register of Pre-trial Investigations (in case of a passport theft on the territory of Ukraine);

4) documents confirming the payment of the administrative fee, the state duty;

5) certificate of registration of homeless persons (for homeless persons);

6) certificate of registration of IDPs (for IDPs);

7) judgement of the establishment of identity (for persons who were not identified by the results of the procedure of establishment of identity);

8) a certificate on the assignment of the registration number of the taxpayer’s account card from the State Register of natural taxpayers or the notification of refusal of its acceptance (for persons who, due to their religious beliefs, refuse to accept the specified number);

9) certificate of registration of the place of residence of the person (if any);

10) birth certificates of children (if any);

11) marriage certificate, marriage dissolution certificate, judgement of marriage dissolution (if any);

12) certificate of change of name (if any).

If a person is recognized by the court as partially incapacitated or incapacitated, the documents and the application form shall be submitted by one of its legal representatives.

The original documents (except for the documents confirming the payment of the administrative fee and the state duty) shall be returned to a person or his/her legal representative after the application form is issued.

For the submission of documents by a person who cannot move on his own due to a long-term health disorder (as evidenced by the medical assessment of the healthcare institution), according to the worker or his/her legal representative`s application leavimg the place of residence or undergoing treatment would be carried out.

To undergo the procedure of establishment of identity it is recommended to submit any documents or their copies with any photo and written explanation in an arbitrary form with the information about addresses of the place of residence, study, place of employment, information about parents or other relatives who will be involved in the identification process. Documents (copies) supporting the above facts (if any) shall be enclosed to the written request.

If the results of the procedure do not identify the person, you must turn to the court.

Time for completion and cost of service:

  • up to 20 working days – 345 UAH
  • not later than 10 working days – 471 UAH.

State duty – 34 UAH (2tax-exempt minimum incomes).

In the case of the identification procedure, the period of consideration of the application form may be extended up to two months.

What to do if a passport in the form of a credit card is lost?

The same, except that persons who are unable to move on their own, they should add a 10×15 cm photo that shall comply with the requirements of Doc 9303 International Civil Aviation Organization (ICAO) recommendations.

Time for completion and cost of service:

  • up to 20 working days – 345 UAH
  • not later than 10 working days – 471 UAH.

State duty – 34 UAH.

What should you do if you need to make adjustments in your passport or exchange it for a new one?

The passport can be updated or exchanged if the information is changed or an error is found.

In order to make adjustments or exchange a passport, you need to add the basic documents (listed above), as well as the supporting documents that confirm the statement for entering additional information: certificate of change of name, birth certificate of a child, marriage certificate, marriage dissolution certificate, judgement of marriage dissolution, certificate of registration of the place of residence of the person, etc.

Persons aged 25 and 45 must attach a new photo if they wish to leave the old passport (in order to do this youhave to apply no later than one month prior to the expiry date of the passport with the old photo), or, if desired, they may change a passport in booklet form to the passport in the form of an ID card.

After acceptance of the documents, the passport has to be exchanged (except cases where the passport is not suitable for further use), upon written request, may be returned to a person for the period of preparation of the new document. While receiving a new passport, the old document is withdrawn.

  • not later than 10 working days – 471 UAH.

State duty – 34 UAH.

What should you do ifyour passport as a credit card expires?

The same that it said in the previous paragraph.

In case of submission of documents within one month prior to the expiration date of the passport, after acceptance the documents, a passport is returned to a person and surrendered when receiving a new passport.

  • not later than 10 working days – 471 UAH.

State duty – 34 UAH.
